Face Lift

Dr. Kun,

I am interested in a face lift, but I'm concerned about the cost and time away from work. Is there an alternative to a full face lift that can provide positive results? Do you have samples of before & after photos that I can see, because some of the options I've seen online, sound "too good to be true." Though expense is a stumbling block for me, we're talking about my face and I don't want to be foolish in seeking a youthful look only to end up with an unbalanced or "freaky" looking mess.

First off, your concerns are common, so though I sense your embarrassment by your anonymous inquiry, I assure you that you are not alone. We've completed countless procedures we call the "ONE HOUR MAGIC LIFT." Ask Dr. Kim About Rhinoplasty Revision Surgery

I underwent elective rhinoplasty surgery in my early twenties. At the time, the result seemed better than my natural nose, but I've always felt like my "new" nose looked sort of fake--it has very sharp and pointy features and my nostrils look too big for my face with the tip having two boney points. My other facial features are much more subtle and less chiseled, so my nose just doesn't seem to fit my face. Sometimes I feel silly for wanting to change it again, but it just never met my expectations and ten years later, I'm still unhappy. Is there anything I can do?

I'm sure every surgeon's intention is to provide patients with a natural look, but sometimes the result just doesn't match the patient's hopes. Before surgery, I try my best to inform and encourage patients to be realistic. I've helped countless patients get the rhinoplasty results they've desired from the start and I've also helped patients like you who need rhinoplasty revision.

Let's step back for just a minute to explain what rhinoplasty surgery is for those readers who may be unaware and then we can talk about rhinoplasty revision surgery. Rhinoplasty is surgery to the nose to improve its appearance from birth defects, injury, or simply to improve breathing due to a deviated septum/nasal obstruction/chronic sinusitis. Through surgery, we reshape and
enhance the tissue and bones to either reduce or increase the nasal bridge, the slope of the nose, angle or nostrils.

Now, to get to you specifically, Sarah: I sense you feel somewhat unreasonable for wanting to revise your rhinoplasty surgery, so I want to assure you rhinoplasty revision surgery is very common. Try not to be upset with yourself, especially since your initial surgery was elective and not due to an injury. I've done countless revision surgeries of this type and advise patients not
to psychologically beat themselves up and just know they deserve to be happy. If you're not, it's perfectly understandable and acceptable to make a change.

Revision rhinoplasty surgery is different from the primary surgery, because the tissue and cartilage is sometimes over-resected or asymmetrical and the cartilage may be distorted. I take special attention to analyzing the current nasal status and confer with the patient to make sure we're both on the same page and have realistic expectations. By taking that time beforehand, I've had positive results with rhinoplasty revision surgery and my patients have been pleased with the results.

I'm sure we can improve the status of your nose, Sarah. Again, don't feel silly for being unhappy with the results of your initial surgery. Especially, since after ten years, you are still not pleased.

As I’ve aged, I’m noticing that my facial skin is starting to sort of shift and sag, especially around my mouth. I have these lines now from my nose to my mouth—I’ve heard them called “parenthesis”--and I look somewhat puppet-like. This coupled with the frown lines between my eyebrows are really making me appear older and more tired than I feel.

I hate to sound superficial, but I’m really starting to dislike my reflection. Gina,

From what you’ve described, it doesn’t sound like surgery is recommended at this time. There are many nonsurgical alternatives that we can try. I suggest you visit my website and take a look at the different injectable options such as Restylane or Radiesse for the parenthesis and maybe some Botox to relax the furrowed brows.

Without seeing you, I don’t want to make a definite recommendation, but these are usually good choices for smoothing wrinkles and increasing volume to give you that more refreshed look you’re seeking, without surgery--and the results can last anywhere to a few months to over a year depending what we use.

People frequently send me emails requesting my opinion on a particular skin-related problem. It recently occurred to me to ask permission to share these inquiries and my recommendations on this blog in order to offer information to other subscribers who may have similar issues. The following email & answer may be of assistance to those of you struggling with unsightly scarring. I hope it helps.

I was in an accident when I was a teenager which resulted in a thin scar that runs perpendicular to my jaw line about 3 ½ inches. It’s not overly obvious and my husband says he “doesn’t even notice it,” but it bothers me. The skin is raised a bit so I feel it every morning when I apply makeup.

Can you recommend something to lessen the appearance of the scar and to smooth it, so I don’t notice it so much? I’m not sure I’d go as far as to have surgery, so I’m hoping there are other options.

Of course, I’d have to see the scar in person to make a definite recommendation, but generally the appearance of facial scars can be diminished by a technique called dermabrasion. I usually recommend waiting up to a year after injury to see how the body heals on its own. Since your injury occurred a number of years ago and it doesn’t sound like surgery is recommended, we could try dermabrasion.

For lack of a better analogy, dermabrasion is like a controlled sanding down of the top layers of the skin to improve skin contour. Dermabrasion is performed in our outpatient facility and is relatively painless. After the procedure, you’ll feel as though you have a brush-burn and your skin will appear pink for up to eight weeks. You may cover the redness with makeup during the healing process. Depending on the results, an additional dermabrasion may be recommended, but given the reported details of your scarring, it sounds like one treatment would suffice.

Liposuction techniques offered

Liposuction’s original function was to contour the body. Since its beginnings, it has evolved as a useful tool for other cosmetic procedures. It can reduce and contour the unwanted pockets of fat accumulated over the years from the face, neck, arms, tummy and other parts of the body. These kinds of liposuction procedures can accompany facelifts, eyelifts, and browlifts and may include chin or cheek implants to further enhance cosmetic appearance.
How it works

Liposuction eliminates unwanted pockets of fat from neck, arms, tummy or from any other areas of the body. This can usually be performed under local anesthesia with mild sedation. Tumescent micro-cannula liposuction technique is used which at this time is the gold standard by which all other techniques of liposuction is measured. Good candidates for blepharoplasty

Candidates for eyelid surgery, or blepharoplasty, are concerned with droopy eyelids and/or puffiness under their eyes. The blepharoplasty procedure can be performed on the upper or lower eyelids or both; it focuses on eliminating fat, muscle, and excess skin to create a younger looking appearance.

Eyelid surgery patients should be 18 years old or older. Most patients are at least 35 years old, but if droopy or baggy eyelids are a trait that runs in your family, the surgery may be something you would consider at a younger age.

Medical uses for injectables

Botox is well known for its ability to take years off of our faces, but a little known fact is that it also has medical uses.

Treatment for Eye Spasms
One such use is for people afflicted with eye spasm, or blepharospasm and hemi-facial spasms. Drug treatment is not always effective for twitches of the eye and does not always lead to results that last. Dr. Kim can utilize Botox injections which often work very well and are safe to use. Choosing the facelift that's right for you

While facelifts of the past were generally of the one-size-fits-all variety, facelift alternatives are now available to patients seeking a more youthful appearance. A facelift makes loose skin in the neck, cheeks, and jaw line more taut. While this procedure will not remove forehead wrinkles, "crow's feet" or bags under the eye, it may be used in concert with other procedures that eliminate excess fatty tissue.

Traditional Facelift
With a full facelift, incisions are made within the hairline and following the ear’s natural contour. This allows the skin to be pulled back. Small incisions help to make the scars much less noticeable. Sometimes liposuction is added to remove excess tissue from the jowl area. This procedure is typically done in an outpatient surgical setting under general anesthesia. Dramatic results are possible but with 2 weeks to 2 months of recovery may be necessary depending on patients.

After surgery, a dressing is worn for 24 hours and cold compresses are used to reduce swelling. After dressings are taken off, cold compresses are placed on the swollen areas for another 48 hours. Patients greatly limit their movement for the first 1-2 weeks.

The One Hour Facelift
Dr. Kim’s one hour facelift is performed under local anesthesia with light sedation if desired. General anesthesia and its associated risks are eliminated, and the recovery period is drastically shortened. Traditional face lifts were inaccessible to many due to high cost and the lengthy recovery periods—the one hour lift provides a new option. Designed primarily for chin jowls and upper neck, dramatic results are also obtained. Most all of the before and after section of the face lift were done with this type of procedure. Best Option for Healthy Appearance: Healthy Diet

Facial fillers have seen a dramatic increase in popularity in recent years, especially hyaluronic-acid-based fillers, like Restylane, whose use has grew by 35 % last year according to the American Society of Plastic Surgeons. However, the American Society for Dermatologic Surgery cautions that fillers are sometimes used inappropriately.

According to Dr. Rhoda Narins, past president of the ASDS, many celebrities are turning to fillers to conceal the effects of yo-yo dieting and unhealthy eating. "Since the face is the first part of the body to reflect unhealthy eating habits and dieting," she said, "more celebrities are requesting fillers to 'naturally' plump up sunken facial features." Extreme weight loss thins the skin, making the face appear empty, narrow, and aged. Celebrities use fillers to conceal the fact that their super-small figures are unnatural.

Aging Skin and Wrinkle Remedies

Some effects of aging cannot be prevented because they are timed by genetic triggers that are poorly understood. In general, as we age, three things take place: the regeneration process of skin slows down, the dermis (the inner layer of the skin) thins, and we lose subcutaneous fat. All three of these changes have significant consequences in making us look older, but they all can be mediated to some extent.

When we are young, our skin is constantly shedding and renewing itself, but as we age, this process slows down. This leads to skin that is older, has experienced more sun and other factors, leading to dull skin with age spots and other signs of sun damage. The skin appears crepey and gets numerous fine wrinkles. A remedy to this process is to simply accelerate the natural shedding process by using a skin resurfacing technique such as dermabrasion, laser resurfacing, or facial peels.

The dermis is the home of collagen and elastin to give strength and flexibility to the skin. Unfortunately, as we age, this layer of the skin thins. It loses collagen, and the elastin fibers break up. The combination of these factors can make the skin less elastic and more likely to retain folds and creases, what are known as dynamic wrinkles. This type of wrinkle can be remedied by using techniques that stimulate the production of collagen in the dermis, including resurfacing techniques, especially Fraxel. Sometimes, this type of wrinkle is not due to the skin at all, but due to the action of facial muscles. In this case, Botox treatments can remedy the wrinkles.

Subcutaneous fat loss can have several significant effects. It can create hollows in the face, especially under the eyes and in the cheeks. All surgeries have risks, but the risks of plastic surgery can be minimized if you make the right choices. Here are some factors that can help you make sure your facial surgery is as safe as possible:

Make sure you meet your plastic surgeon before agreeing to the surgery. Your surgeon should be prepared to meet with you for as long as it takes to address your concerns, and not be brushing your questions aside to rush off to another appointment.
Your surgeon should put safety first. A surgeon vested in a culture of safety, he or she will take care of many details that you can neither appreciate nor evaluate before the surgery. Gauge your surgeon's responses to questions during your consultation. Is safety at the forefront, or does it seem to be merely an afterthought?
Check your surgeon's reputation, expertise, and experience. Your surgeon should not only be trained and skilled in your procedure, but he or she should have performed the surgery numerous times recently.
Your surgery should take place either in a hospital or in an accredited surgical suite. The accreditation should be done either through the Accreditation Association for Ambulatory Health Care (AAHC) or the Ambulatory Surgical Center Association (ASCA).
You surgeon and support personnel should all be up to date and certified in advanced cardiac life support, and have contingency plans prepared in the event of complications, including a transfer agreement with a nearby hospital.
